授乳
児の身体的健康および認知の発達に対する授乳の長期的な効能はよく知られている。女性は通常少なくとも6ヵ月は母乳育児するよう勧められる。母親の母乳育児の決定に影響を及ぼす可能性のある因子の1つは,授乳中に使用する薬剤の安全性である。注意すべきいくつかの例外はあるが,ほとんどの向精神薬は,授乳が有益であることや有害性のエビデンスがないことから,授乳中の女性でも継続すべきである。しかし,現在のエビデンスから,いくつかの薬剤(以降参照)については,投与が母親に最良の治療選択肢であれば授乳を行わないよう助言すべきであることが示唆されている。
授乳期における向精神薬の安全性データは,主に小規模研究,症例報告,症例集積研究から得られたものである。多くの症例での新生児転帰や乳児転帰の報告は,短期間の急性副作用に限られたものが多い。したがって,ここで言及する向精神薬についての長期的な安全性は保証できない。本項は基になるデータが限られており,またその情報は定期的に更新すべきものであるため,本項の情報は慎重に解釈する必要がある。
乳児への曝露
程度の差はあるが,すべての向精神薬が母乳に移行する。乳児への曝露を直接測定する方法は,むろん乳児の血漿中薬物濃度を測定することであるが,このようなデータはほとんど得られていない。その代わりとして,多くの論文では母乳中濃度と母親の血漿中濃度のみを報告している。母親の抗精神病薬の血漿中濃度は乳児の曝露量推定に有用と考えられる1。母乳中の薬剤濃度から乳児の1日の摂取量を推定することができる(母乳摂取量を150mL/kg/日と仮定)。乳児の体重で調整した摂取量を,母親の体重で調整した投与量の比として表現したものが,相対的乳児投与量(RID)である。RIDの値は推定値であり,どの薬剤についても文献によってかなり幅があるため,目安としてのみ考えるべきである。
一般には,RIDが10%未満の薬剤は授乳中でも安全と考えられている。乳児の血漿中濃度が測定できる場合は,乳児の血漿中濃度が母親の平均血漿中濃度の10%未満であれば,授乳中も安全とすることが提案されている2。
授乳期の向精神薬処方の一般的原則
- 妊娠を考えている女性に向精神薬を処方する場合,授乳における個々の薬剤の安全性を考慮に入れるべきである。
- 授乳中の薬剤の安全性に関する話し合いをできる限り早く行うべきで,妊娠前または妊娠初期が理想的である。妊娠中の薬剤の使用に関する決定では,こういった授乳についての話し合いを加味すべきである。再発のリスクが高いため,妊娠末期または出産後数日間に薬剤を切り替えることは勧められない。
- 妊娠中および出産するまで特定の向精神薬を使用していた母親では,乳児の離脱症状を最小限に抑えられる可能性があるため,授乳中も当該薬剤を継続するのが適切であろう(注意すべき例外については以降を参照)。
- 患者毎に,授乳が母子にもたらす利益と,乳児の薬物曝露のリスクを比較考慮しなければならない。
- 現在処方されている薬剤は授乳期が禁忌でない限り,通常は授乳を中止するのは不適切である。母親の精神疾患の治療が最優先であるため,授乳期が禁忌の場合は治療を中止するのではなく,母親に人工乳の使用を助言すべきである。
- 産後に薬剤の投与を開始する場合:
- 治療に対する以前の反応を考慮することが重要である。
- 乳児の血漿中濃度または RID が高いことが報告されている向精神薬の使用は控えるのが最善である。
- 薬剤の半減期を考慮することが重要である。半減期が長い薬剤は母乳や乳児の血清中に蓄積するおそれがある。
- 薬剤クリアランス能力は,新生児・乳児と成人で異なる。さらに,早産児や腎臓・肝臓・心臓または神経に障害のある乳児は,薬剤曝露によるリスクが高い。
- その薬剤特有の副作用が乳児に現れていないか,また授乳パターンや乳児の成長・発育状況に異常がないか,継続的にモニタリングすべきである。
- 副作用が認められるときまたは毒性が疑われるときには,乳児の血漿中濃度をモニタリングすべきである。
- 鎮静薬を使用している女性は,ベッドで授乳すると寝入ってしまい乳児の上に寝返りをうって乳児に低酸素症のリスクをもたらす可能性があるため,添い寝しながら授乳しないよう強く指導すべきである。
- 鎮静薬は,児と触れ合う母親の能力に影響を及ぼす可能性がある。鎮静薬を使用している女性にはこの影響についてモニタリングを行うべきである。
- 可能な限り以下の原則を守る:
- 最小有効用量を使用する。
- 多剤併用は避ける。
- 妊娠中に処方された投与レジメンを継続する。
表7.2に授乳中の薬剤使用に関する推奨事項の要約を示す。個々の薬剤の情報は表7.3-7.7にある。

通常は,妊娠中に処方された抗うつ薬を継続することが推奨される。授乳のため出産後に薬剤を切り替えることは一般には賢明とはいえない。表7.3は,出産後に治療を開始する際の目安として用いるべきである。症例毎に,治療に対する以前の反応(および反応の欠如)を考慮しなければならない。

てんかん罹患女性から生まれた小児を前方視的に調査した研究では,抗てんかん薬使用中に授乳をしても,月齢6-36ヵ月の乳児の発達に有害な影響はなかった。この研究では,抗てんかん薬に子宮内で曝露された後に授乳でも曝露された小児の転帰を評価し,曝露されなかった小児と比較した 抗てんかん薬に母乳を介して曝露した乳児199例を検討した研究では,母乳を摂取した乳児と摂取しなかった乳児の3歳時のIQに差は認められなかった。これらの乳児は子宮内で抗てんかん薬に曝露していた 乳児181例を対象とした研究では,母乳を介した抗てんかん薬の曝露によりIQに有害な影響は認められないと結論付けられた |

表7.7には,2020年半ばに発表された入手可能なデータをもとに,授乳中の精神刺激薬に関する情報を薬剤毎に示している。授乳中の精神刺激薬に関する各製薬会社の注意事項は,それぞれの薬剤の製品概要および欧州公開医薬品審査報告書に記載されている。表7.7では,こういった注意事項は(情報価値に乏しいことが多いため)含めず,一次資料の方を使用している。通常は,妊娠中にも処方された薬剤を継続することが推奨される。授乳のため出産後に薬剤を切り替えることは一般には賢明とはいえない。表7.7は,出産後に治療を開始する際の目安として用いるべきである。症例毎に,治療に対する以前の反応(および反応の欠如)を考慮しなければならない。
