SSRIは一般的に心疾患があっても推奨されるが,循環器系の薬剤を併用する場合は抗血小板作用やシトクロムを介する相互作用に注意しなければならない。ミルタザピンは適切な選択肢とされてきたが32,解析ではミルタザピンも出血性疾患と関連することが示唆されている63。
SSRIは心筋梗塞の予防効果を示す可能性があり64, 65,未治療のうつ病は心血管疾患の予後を悪化させる66。心筋梗塞(MI)後にSSRIやミルタザピンを投与すると,死亡率に対して無影響ないしは有益な効果をもたらす67。したがって,MI後でもSSRIによるうつ病の治療は控えるべきではない。MI後のうつ病治療による予防作用は抗うつ薬の投与に関連し,抗うつ薬の抗凝固作用,または不整脈の頻度を間接的に減少させる作用によるもののようである45, 68。この点において認知行動療法(CBT)は効果がないかもしれない69。ただし,SSRIの抗血小板作用には悪影響もあることに注意すべきである(本Chapterの「SSRIと出血」の項を参照)。
