クロザピン:よくみられる副作用
副作用 | 経過 | 対応 |
鎮静 | 開始後数ヵ月間に生じる。持続することもあるが,通常はある程度軽減する | 朝の用量を少なくする。朝の目覚めに問題がある場合は,晩の服用時間を早くする 可能であればクロザピンを減量する 精神刺激薬(メチルフェニデート1)およびベタヒスチン2が奏効したという症例報告があるが,長期のデータはない。モダフィニルは効果がないようである3 |
流涎過多 | 開始後数ヵ月間に生じる。通常は持続するが,軽減することもある。しばしば夜間に問題となる | hyoscine 300mcgを口の中で吸って飲み込む。1日3回まで。他にも多くの選択肢があるので,流涎過多に関する項を参照のこと。注:抗コリン薬は便秘と認知機能を悪化させる |
便秘 | 最もリスクが高いのは開始後4ヵ月である4。通常は持続するため,継続的なモニタリング/治療が必要である | クロザピン開始前にリスクについて説明し,定期的にスクリーニングを行い,食物繊維や水分を十分に摂っているか,十分な運動を行っているかを確認する。刺激性緩下剤(センナ)が第一選択であり,必要に応じて便軟化剤(docusate)および/または浸透圧性緩下剤(マクロゴール)を追加する5。根本原因は胃運動低下であるため,膨張性緩下剤は通常は避けるべきである。他に便秘の誘因となりうる薬剤を中止し,可能であればクロザピンを減量する。便秘から死に至ることもあるので,効果的な治療または予防が必須である4, 6-9。クロザピンによる便秘の項を参照のこと |
低血圧 | 開始後4週間に生じる | 立ち上がるときは時間をかけるように助言する クロザピンを減量するか増量ペースを落とす。水分摂取量を2L/日に増加させる10。重度の場合はmoclobemide,Bovril11,フルドロコルチゾン,デスモプレシン,腹帯の使用を検討する10。長期的には体重増加から高血圧に至る可能性がある |
高血圧12 | 開始後4週間に生じるが,長引く場合もある | 慎重にモニタリングし,必要であれば増量のペースを落とす。降圧療法が必要になることもある13 |
頻脈 | 開始後4週間に生じるが,持続することもある | 治療開始後にはよくみられる症状で,通常は良性である。用量に関連することもある14。安静時にも持続したり,発熱,低血圧,胸痛を伴ったりする場合は心筋炎が疑われる15, 16(心血管系の副作用に関する項を参照)。循環器専門医に相談すべきである。胸痛や心不全に随伴して頻脈が起こるようであればクロザピンは中止する。良性の洞性頻脈であれば,ビソプロロール17またはアテノロール18で治療できるが,エビデンスは不足している19, 20。低血圧や禁忌のためβ遮断薬が使用できない場合には,イバブラジンを使用してもよい21。頻脈が長引くと,それ自体が心筋症22やその他の心血管系合併症を促進する可能性があるので要注意である10 |
体重増加 | 通常は開始後1年間に起こるが,持続することもある | 食事指導は必須である。体重が増加し始める前に指導すると効果がより高いかもしれない 体重増加はよくみられる副作用であり,著明なことも多い(開始後10週間で4.5kgの増加23)。多くの対処法があるので,体重増加の治療に関する項を参照のこと |
発熱24 | 開始後4週間に生じる | クロザピンは炎症反応(C反応性蛋白やインターロイキン625,好酸球の増加)を誘発する25-27。paracetamol(アセトアミノフェン)を投与するが,好中球減少についてFBCをチェックすること。増量のペースを下げる28。血液障害に関連する発熱は少ないが29,心筋炎,神経遮断薬悪性症候群(NMS),肺炎やその他の比較的稀なタイプの炎症性臓器障害には注意する(稀な副作用に関する項を参照) |
けいれん発作30 | いつでも起こる可能性がある31 | 用量,血漿中濃度,増量のペースが速いことに関連する32。クロザピンを高用量(≧500mg/日)で用いている,もしくは血漿中濃度が500mcg/L以上と高い場合は,予防的にトピラマート,ラモトリギン,ガバペンチン,バルプロ酸*の投与を考慮する。ただし,1,300mcg/L未満の場合のけいれん発作リスク(20例に1例)は予防的投与を支持しないという見解もある33。けいれん発作が起きた場合はクロザピンを1日中止し,用量を半分にして再開し,抗けいれん薬を投与する**。クロザピン投与中はEEG異常がよくみられる34, 35 |
悪心 | 開始後6週間に生じる | 制吐薬を処方してもよい。EPSの既往がある場合は,プロクロルペラジンやメトクロプラミドは避ける。心疾患リスクやQTc延長がある場合には,ドンペリドンは避ける。オンダンセトロンは適切な選択肢であるが,便秘を悪化させる可能性がある。心筋炎の症状として悪心と嘔吐のみが現れた1症例が報告されている36 |
夜尿 | いつでも起こる可能性がある | 減量するか,投与スケジュールを調節して時間帯により深い鎮静がかかることを避ける。就寝前の水分摂取を避ける。夜間の予定排尿を検討する。自然軽快することもあるが37,月,年単位で持続することもある38。クロザピン服用患者5例に1例の割合で起こる可能性がある39。重症の場合は,通常,デスモプレシン点鼻薬(10-20mcg就寝前投与)が有効であるが40,リスクがないわけではなく,低ナトリウム血症が生じる可能性がある41。抗コリン薬は有効かもしれないが42,支持するエビデンスは弱く,便秘や鎮静が悪化する可能性がある。エフェドリン43,プソイドエフェドリン44,アリピプラゾール45, 46も用いられている |
胃食道逆流症(GERD)47, 48 | いつでも起こる | プロトンポンプ阻害薬がしばしば処方されるが,CYP1A2を誘導するものもあり,好中球減少および無顆粒球症のリスクを増加させる可能性がある49, 50。クロザピンはH2受容体拮抗薬であり,GERDと関連する理由は不明である51 |
ミオクローヌス32, 52-54 | 増量中または血漿中濃度の増加中 | この後に,全般性強直間代発作が起こることがある。用量を減量する。抗てんかん薬は有効かもしれず,けいれん発作への進行の可能性を軽減する。バルプロ酸が第一選択である。ラモトリギンはある種のミオクローヌスを悪化させる可能性がある |
肺炎55-62 | 通常は開始後初期であるが,いつでも起こる可能性がある | 唾液の誤嚥から起こる可能性がある(これが,肺炎が時に用量依存的に生じる理由である可能性がある63, 64)。非常に稀ではあるが,便秘も誘因となる65。肺炎は,クロザピン投与を受けている患者の間ではよくみられる死因である56。クロザピン投与中は一般的に感染症の発生率が高いと考えられ66,抗生物質の使用も増える67。呼吸器感染症によりクロザピン濃度が上昇する可能性があることに注意する68-71(感染中は喫煙しないことが多いため,濃度上昇はこの交絡因子が原因であると考えられるが,炎症によるCYP1A2活性の低下が原因である可能性もある72, 73)。クロザピンは肺炎が軽快したら問題なく投与を継続できることが多いが,再発の可能性も高い74-76 |
*通常の用量は1,000-2,000mg/日である。血漿中濃度は投与量決定のおおまかな指標として役立つ可能性がある。50-100mg/Lを目標とする。徐放性製剤(Epilim Chrono)はコンプライアンス改善に役立つかもしれない。1日1回投与が可能であり,忍容性がより高い可能性がある。
**統合失調感情障害の場合にはバルプロ酸を使用する。妊娠可能年齢の女性の場合,クロザピンが奏効しない場合,または陰性症状が持続する場合にはラモトリギンを使用する。体重を減らす必要がある場合にはトピラマートを使用する(認知機能関連の副作用に注意する)。他の抗けいれん薬に対する忍容性が乏しい場合にはガバペンチンを使用する32。

クロザピンは,特に治療初期において多彩な副作用をもたらす可能性があることから,患者と副作用の種類や徴候,対処方法などについて逐一共有することが重要である。副作用の評価においては,自記式評価尺度であるGlasgow Antipsychotic Side-effects Scale for Clozapine 日本語版(GASS-C-J)の活用も有効である(Kitagawa et al. PLoS One 2020)。
