向精神薬の血漿中濃度モニタリング
血漿中の薬物濃度,すなわち「血漿中濃度」のモニタリングに関しては,混乱や誤解が多い。薬物濃度モニタリングは,適切に行われれば治療を最適化して服薬アドヒアランスを確実なものにするうえで大きく役立つ。しかし,精神科領域においても他の領域と同様に,血漿中濃度測定が明確な根拠なしに不適切に行われていることがしばしばある1。また,薬物血中濃度モニタリングが十分に活用されていないこともある。
採血して血漿中濃度を測定する前に,以下の基準を満たしているか確認する。
- 臨床的に有用な分析方法はあるか?
一部の薬剤でのみ可能である。分析方法は臨床的に妥当と認められたもので,分析結果が臨床的に有用な時間内で利用できるものでなければならない。分析を依頼する検査機関に確認すること。
- 薬剤が「定常状態」にあるか?
通常,血漿中濃度の測定は,薬剤が定常状態に達した後に検体が採取された場合のみ意味がある。定常状態に達するには半減期の4-5倍の時間を要する。ただし,過量服薬の疑いがある場合は明らかに例外である。このような状況では,定常状態に達しているかどうかは問題ではない。血中濃度を増量の指標として用いる場合(クロザピン等)も例外である。
- 検体採取のタイミングは正しいか?
すべての薬剤ではないにしても,多くの薬剤において,検体の採取時間は非常に重要である。もし推奨された採取時間が服用後12時間であるならば,可能なら服用から11-13時間後,もしやむをえない場合は,服用から10-14時間後に行う必要がある。クロザピン濃度測定のための検体を,予定された検体採取時間である服用後12時間の前後1および2時間で採取した研究では,クロザピン血中濃度の変動の平均は10%未満であったが,一部の患者では血中濃度が50%以上変動していた2。したがって,もし検体採取が予定時間の前後1-2時間以内に行われなかった場合は,血中濃度測定によって情報が得られるどころか,誤った判断を招いてしまう可能性がある。予定時間に可能な限り近い時点で検体を採取するように常に心がけること。言うまでもなく,薬物の毒性が疑われる場合は,予定されたタイミングを無視して直ちに検体を採取すること。
トラフあるいは「服用前」の検体が必要な場合は,次の服用の直前に血液を採取すべきである。いかなる状況でも,検体を採取するために次の服用を1時間もしくは(場合によっては)2時間以上遅らせてはならない。これ以上遅らせると,誤った(通常の服用パターンでの濃度よりも低い)結果が出ることは避けられず,不適切な処方量増加につながりうる。半減期が長い薬剤(例:オランザピン,アリピプラゾール)では検体採取の時間はあまり重要ではないが,処方者は常に検体採取の時間と最終服用の時間を最低限記録すべきである。これは,いくら強調してもしすぎることはない重要事項である。
- 検査値は固有の意味をもっているか?
血漿中濃度に目標域はあるだろうか? 目標域がある場合は,(適切な時間に採取された検体の)血漿中濃度は処方を行う際の有益な指針となる。一般的に認められた目標域がない場合,血漿中濃度は服薬アドヒアランス,または潜在的な毒性を示唆するだけである。しかしながら,もし検体を服薬アドヒアランスの確認に用いるとしても,血漿中濃度が0というのは過去数日間,薬剤を服用しなかったことを示すにすぎないということは理解しておくべきである。また血漿中濃度が0より高い場合は,不規則な服薬アドヒアランス,完全な服薬アドヒアランス,あるいは長期間服薬していなかったが最近になって処方通りに服薬して服薬アドヒアランスを偽装している状況等を意味すると考えられる。目標域には限界があることにも注意する必要がある。患者は目標域以下でも反応しうるし,目標域以上でも忍容しうる。そのうえ,各検査機関での目標域は特別な理由なく大きく異なることがある。
- 血漿中濃度の測定に明確な理由があるか?
以下の理由のみが妥当である。- 服薬アドヒアランスを確認するため(ただし上記参照)
- 毒性が疑われる場合
- 薬物動態学的相互作用が疑われる場合
- 臨床効果を直接評価するのが困難な場合(かつ血漿中濃度の目標域が設定されている場合)
- 薬剤の治療域が狭く,毒性をきたす可能性が高い場合
検査結果の解釈
検査値の解釈の基本原則は,確固たる臨床観察を背景とした検査結果である場合にのみ成り立つものである(「数値ではなく,本人を診る必要がある」)。例えば,血漿中濃度が一般的に認められた目標域より低いが,薬剤に適切に反応している場合は,通常は処方量を増量すべきではない。血漿中濃度は目標域内にあるが,忍容不能な副作用がみられる場合は,処方量を減量するのが適切だろう。
検査結果が前回の血漿中濃度と大きく異なる場合は,通常は再検査するのが望ましい。処方量,服薬時間,最近の服薬アドヒアランスをチェックし,特に正しいタイミングで検体が採取されているか,あるいは少なくとも検体採取のタイミングがわかっているかを確認する。多くの異常値は検体を採取したタイミングの違いによるものである。
目標域について
精神科では,向精神薬濃度の目標域はある程度慎重に取り扱うべきである。反応に関連する濃度の範囲を確立することは,臨床試験における非反応例(血中濃度を問わず反応を示さない例)の存在や,プラセボ反応例および自然寛解例(血中濃度を問わず反応を示す例)の存在によって困難となっている。副作用に基づく目標域の確立は,時間の経過とともに耐性が生じるために困難となっている。したがって,目標域の設定を目的とした研究の大半には,「シグナル」と同じだけの「ノイズ」があり,最終的に結果は広い近似範囲となる。
興味深いことに,実臨床において反応に関連する薬物濃度には,公表されている目標域とのかなり密接な相関が認められる3。薬物濃度の下位四分位(25パーセンタイル)は通常,目標域の下限に近く,上位四分位(75パーセンタイル)は上限値付近にあるが,通常は上限値より低い。おおまかに言うと,患者の25%前後が目標域未満でも反応を示し,最大25%が目標域を超える血中濃度でも忍容できるということになる(表11.1)。

amisulpride
amisulprideの血漿中濃度は投与量に密接に関連しているが,ルーチンの血漿中濃度モニタリングが推奨されるほどの変動はない。女性では高値となるが25-27,治療反応性の点でも副作用の点でも臨床的な意味はほとんどないようである。臨床的な効果が得られる(トラフの)閾値は約100μg/L28とされ,また,いくつかの研究において効果が得られた患者の血漿中濃度の平均値は367μg/L27であった。副作用[特に錐体外路症状(EPS)]は平均血漿中濃度336μg/L25,377μg/L28,395μg/L26で観察された。320μg/L以下の閾値でEPSの回避を見込めることが示されている28。あるレビュー29では,最適な臨床効果と副作用の回避を見込める血漿中濃度は200-320μg/L程度とされたが,最近のコンセンサス声明30では,目標域100-320μg/Lとされている。用量200mg/日で十分に血中濃度100μg/Lに達するため31,この下限値は確実な治療効果を得るにはおそらく低すぎる。高齢の精神病症例では,血漿中濃度20-60μg/Lで最適なD2受容体占拠率と臨床効果が得られるであろうことが示唆されている32, 33。
臨床では「治療域」の血漿中濃度を示すケースは少数であるため(アドヒアランス不良が原因と考えられる34),血漿中濃度モニタリングにはある程度の有益性はあるかもしれない。しかしながら,amisulprideの血漿中濃度モニタリングはほとんど行われず,amisulprideの分析を行う検査機関は非常に少ない。用量-反応関係は(少なくとも臨床試験では)十分確立されているので,承認されている処方量の範囲では血漿採取の必要はなく(ただし,高齢者では50-100mg/日で十分な可能性に留意する),副作用は通常は用量調節のみで十分にコントロールできる。血漿中濃度モニタリングは,臨床効果が不十分な場合,アドヒアランスが疑問視される場合,あるいは薬物相互作用や身体的疾患のために副作用が起こりやすい場合に行うとよい。
アリピプラゾール
アリピプラゾールの血漿中濃度モニタリングは,臨床では時折行われる。アリピプラゾールの用量-反応関係は十分確立されており,約10mg/日以上の用量では臨床効果とドパミンD2受容体占拠率がプラトーに達することが知られている35。アリピプラゾールとその代謝物,および両者(親化合物と代謝物)の血漿中濃度は用量と強い線形関係を示すので,投与量からおおよその血漿中濃度を予測することが,ある程度可能である36。最適な臨床効果を示唆する血漿中濃度の目標域は146-254μg/L37,150-300μg/L38とされており,副作用は210μg/L超で観察されている38。アリピプラゾールの血漿中濃度の個人間変動が観察されているが十分に検討されておらず,性別の影響はほとんどないと思われる39, 40。年齢,代謝酵素の遺伝子型,相互作用する薬剤等が血漿中濃度の個人間変動の要因と推定される38-41。アリピプラゾールの目標域は,150-210μg/L36と推定されている。デポ剤で1ヵ月に300-400mgの投与が行われている場合には,この範囲はほぼ達成されている42。一部の専門家は臨床効果が得られる閾値をより低く100μg/L30と提案しているが,この血漿中濃度は通常10mg/日の経口投与で得られる31, 43。
クロザピン
クロザピンの血漿中濃度はほとんどの場合1日量に関連するが44,血漿中濃度は多様であり,正確な予測は困難である。血漿中濃度は,一般に若年患者,男性45,喫煙者46で低く,アジア人で高い47。東アジア48, 49,インド50,バングラデシュ51の人々では必要とされるクロザピン投与量がはるかに少ない。また,東アジア人ではクロザピンの代謝活性能力が低い人の割合が高い52, 53。服用者の要因によってクロザピン濃度を予測するアルゴリズムがいくつか作成されており,それらの使用が強く推奨されている54。しかし,アルゴリズムではアドヒアランスの変化,炎症55,感染56, 57等の他の影響までは説明できない。
クロザピンに対して急性反応を示す血漿中濃度閾値は,200μg/L58,350μg/L59-61,370μg/L62,420μg/L63,504μg/L64,550μg/L65と推定されている。限られたデータにより,再発を防ぐには200μg/L以上が必要であることが示唆されている66。クロザピンの血漿中濃度の大きな変動で再発が予測されることもある67。クロザピンの血漿中濃度の個人内変動はよくみられ,時間の経過とともに濃度は若干低下する傾向があるが68,ある研究ではノルクロザピン濃度のみが低下することが示唆されている69。
反応性の閾値に関してはこのようにやや異なる推定がなされているが,治療を最適化するうえで血漿中濃度は有用である。クロザピンに反応を示さない症例では,血漿中濃度が350-600μg/L(上記でのコンセンサス範囲内30)となるように処方量を調整すべきである。クロザピン不耐例では,血漿中濃度がこの範囲になるよう処方量を減量するとよいだろう。クロザピンの目標域の上限は明確にされていない。上限については2つの要素,すなわちこの範囲を超えると追加的な治療効果が得られないレベルと,許容されない毒性/忍容性が生じるレベルを考慮しなければならない。血漿中濃度は脳波(EEG)上の変化を予測すると考えられ70, 71,1,000μg/Lを超える患者はけいれん発作を起こしやすいため72,血漿中濃度はこの値より十分低く保つべきである。クロザピン関連の他の非神経学的副作用もやはり血漿中濃度に関連していると考えられる73。濃度の上限は約600-800μg/Lが提案されている74。
クロザピンの目標域に上限を設定すると,承認されている用量内での有意義な増量が妨げられる可能性があることにも留意しなければならない。血漿中濃度の測定が一般的に行われるようになる前は,クロザピン投与量が600mg/日以上になるとバルプロ酸を加え,最大900mg/日まで処方されることがかなり頻繁にあった。このような高用量の投与が,血漿中濃度が許容された閾値をすでに超えている患者にとって有益であるかどうかはいまだ不明である。それでも血漿中濃度が600μg/Lを超える場合には(明確なエビデンスに基づいた閾値74ではなく,これまでの何度かの推奨に基づく閾値),けいれん発作やミオクローヌスの予防のために抗てんかん薬を投与するのは賢明であり,1,000μg/Lに達する場合は必ず抗てんかん薬を投与する。
ノルクロザピンはクロザピンの主要代謝産物である。クロザピンとノルクロザピンの比率の集団平均は1.25であるが75,個人差があるであろう。長期投与する場合,比率を個々の患者で一定に保つ必要がある。比率の低下では酵素誘導が疑われ,比率の上昇では酵素阻害,トラフ以外の検体,または直近の服薬忘れが疑われる。しかし,早期の検体ではクロザピン濃度が比較的高く,後期の検体ではノルクロザピン濃度が高いため,検体採取の時間によってクロザピン/ノルクロザピンの比率は急激に変化する2。クロザピンは高用量で代謝が飽和する可能性がある。血漿中濃度の上昇とともにクロザピンとノルクロザピンの比率が高くなり,飽和状態が示唆される76-78。フルボキサミンとの相互作用でも,CYP1A2を介したノルクロザピンへの代謝が弱まる可能性が示唆されている79。最近のシステマティック・レビューでは,クロザピン/ノルクロザピンの比率を知っていても臨床的に有用ではないと結論付けられた80。
オランザピン
オランザピンの血漿中濃度は,1日量と線形関係を示す81。しかし個人差が大きく82,女性64,非喫煙者83,酵素阻害薬服用中の患者83, 84では血中濃度が高くなる。1日1回の投与での統合失調症における反応性の閾値レベルは,トラフ時で9.3μg/L85,服用12時間後で23.2μg/L64,服用平均13.5時間後で23μg/L86とされている。服用12時間後の濃度を約40μg/Lより高くしても,低濃度と比較してそれ以上の治療的な有益性はないことを示唆するエビデンスがある87。重度の毒性は稀であるが,100μg/Lを超える場合には毒性が生じる可能性があり,160μg/Lを超える濃度では死亡例もある88(ただし他剤や身体的な要因が関連している場合もある)。治療目標域は統合失調症では20-40μg/L(服用12時間後)と提唱されており89,躁病においてもおそらく同様の値である90。この目標域は最近20-80μg/Lに広げられているが91, 92,その理由は不明である。
特に血漿中濃度が20μg/Lを超えると著明な体重増加が生じやすい93。便秘,口渇,頻脈も血漿中濃度と関係していると考えられている94。
実臨床では,オランザピンの用量は主に反応性と忍容性で決定すべきである。しかし英国における検査結果の調査では,1日20mgを服用している症例の約20%は治療域以下の血漿中濃度しか得られておらず,40%以上の患者が40μg/Lを超える血漿中濃度であることが示唆された95。したがって,アドヒアランス不良が疑われる場合,忍容性がない場合,承認されている最大用量でも効果がみられない場合は,血漿中濃度の測定が有用かもしれない。反応不良で血漿中濃度が20μg/L未満の場合には,12時間後の血漿中濃度が20-40μg/Lになるまで増量してよいかもしれない。一方,反応が良好で忍容性がない場合には,血漿中濃度が40μg/L未満になるまで,まずは減量すべきである。血漿中濃度には,用量の変化に比例した変化が認められる96。血中濃度が40-80μg/Lになるまで投与量を増やす場合もあるが,他に選択肢が残されていない場合のみとする。
クエチアピン
クエチアピンの用量と血漿中トラフ濃度との相関は弱い97。150-800mg/日の用量内での血漿中濃度の平均値は27-387μg/Lであったと報告されているが98-103,最高値と最低値は必ずしも最大投与量と最小投与量によって得られるわけではない。年齢,性別,そして併用薬剤が薬物血中濃度モニタリング(TDM)研究でみられる著明な個人差に影響している可能性があり,女性103, 104,高齢102, 103,CYP3A4阻害薬98, 102, 103がクエチアピン濃度の上昇と関連すると考えられる。これらの報告は結果が一貫していないため104,これらの因子のみに基づいて定期的な血漿中濃度モニタリングを行うことは推奨されない。血漿中濃度は投与量によって大きく変化するが,目指すべき目標治療域を示すエビデンスは不十分である(ただし,目標範囲として100-500μg/Lが提唱されている105)。したがって,血漿中濃度モニタリングにはほとんど意味がないと考えられる。さらに,クエチアピン代謝物には大きな治療効果があるが,その濃度と親化合物の濃度との関連性は低い106。
クエチアピン濃度の関連性に関する最近の報告のほとんどは,トラフ時の検体の分析から得られたものである。クエチアピンは半減期が短いため,トラフ濃度は用量やピーク値に関わらず比較的低濃度に収まる傾向がある。よって,用量や臨床効果にはピーク値がより密接に関連するかもしれないが97,ピーク値のモニタリングは目標域が確立されていないため,現在のところ正当化されない。興味深いことに,境界性パーソナリティ障害または薬剤誘発性の精神病を有する患者におけるクエチアピンの研究では,治療反応と投与12時間後の血漿中濃度に線形関係が認められた104。ピーク値からトラフ値への変動は即放性(IR)製剤(おおよそ最大4,000μg/Lから0)の方が徐放性製剤(おおよそ最大3,000μg/Lから100μg/L前後)より大きい43。
クエチアピンは確立された用量-反応関係があり,承認された用量以上でも忍容性に優れているようである107。実臨床では,症例の反応性と忍容性に基づいて用量を調整すべきである。
リスペリドン
リスペリドンの治療域は活性体(リスペリドン+9-OHリスペリドン)の値が20-60μg/Lということで一般に合意が得られているが91, 108, 109,その他の範囲(25-150μg/Lおよび25-80μg/L)も提唱されている110。20-60μg/Lの血漿中濃度は通常,3-6mg/日の経口投与で得られる108, 111-113。およそ20μg/Lの血漿中濃度で線状体ドパミンD2受容体占拠率は約65%(急性期の治療効果を生じる最小値)とされている109, 114。
リスペリドン持効性注射剤(RLAI,25mg/2週間)で平均4.4-22.7μg/Lの血漿中濃度が得られると考えられる112。この用量におけるドパミンD2受容体占拠率は推定で25-71%と幅がある109, 115, 116。これらの平均値は個人差が非常に大きく,平均値以上の数値を示す患者もいる。それでも,これらのデータは25mg/2週間での有効性に疑問を投げかけるが112,抗精神病薬の持効性製剤で達成される血漿中濃度およびドパミンD2受容体占拠率が見かけ上は治療域以下でも実際には有効であるというエビデンスもあることに注意すべきである117。実際に,長期投与において再発を防ぐうえで,高いドパミン受容体占拠率の維持は(急性効果とは対照的に)不要であるというエビデンスが増え続けている118-120。
ただし,RLAIの投与を受けている症例を対象とした解析では121,症例の50%で血漿中濃度が20μg/Lを下回り,10%でリスペリドン/9-OHリスペリドンが検出されなかったという,気がかりな結果が報告されている。したがって,RLAI投与時の薬物血中濃度モニタリングは臨床的に意味があるかもしれないが,むしろ持効性注射剤の目的を損なうことになる。
月1回投与のパルミチン酸パリペリドンLAIに関する限られたデータでは,得られる血漿中濃度は標準の負荷用量で25-45μg/L,定常状態では100mg/月の用量で10-25μg/L,150mg/月の用量で15-35μg/Lであることが示唆されている122。血漿中濃度は投与開始後1年間に徐々に上昇し,35μg/L前後(平均用量138mg/月)に達した後123,一定になると考えられる124。3ヵ月に1回投与の注射剤では,定常状態で得られる血漿中濃度は,525mg/3ヵ月の用量で30-55μg/L,350mg/3ヵ月の用量で25-55μg/L,263mg/3ヵ月の用量で20-35μg/Lである125。
表11.2に記載した目標域は,有用性がいくぶん疑わしく,場合によっては単に臨床で使用されている値の範囲を示したものである。これらの薬剤の分析は専門機関でしか行われていない可能性がある。
