抗うつ薬と高プロラクチン血症
プロラクチンの分泌は内因性ドパミンでコントロールされるが,5-HT1C受容体,5-HT2受容体の刺激を介してセロトニンによっても間接的に調節される1, 2。(症状の有無を問わず)抗うつ薬による長期的な血漿プロラクチンの上昇はごく稀にみられる3。抗うつ薬による高プロラクチン血症は,実際に起こったとしてもわずかな上昇が一時的にみられるだけのことが多く4,症状が出現するのは非常に稀である。SSRIの使用と乳癌には関連はない5。
プロラクチンのルーチンのモニタリングは推奨されないが,症状から高プロラクチン血症が疑われる場合には,血漿プロラクチンの測定が必須である。症候性の高プロラクチン血症が確認された場合はミルタザピンへの切り替えが推奨されるが(以下を参照),他のSSRIに切り替えても症状が消失するというエビデンスもある6, 7。
