抗精神病薬による体重増加の治療
体重増加は,ほぼすべての抗精神病薬で起こる重要な副作用であり,患者の自己イメージ,罹患率および死亡率に影響を及ぼすことは明らかである。したがって,体重増加の予防と治療が臨床においてかなり急を要する課題となる。
モニタリング
抗精神病薬の投与を開始もしくは変更した場合は,最低でも体重を測定し,明確に記録すべきである。理想的には,体格指数(BMI)と腹囲も記録すべきである1, 2。治療初期は,少なくとも最初の6ヵ月は毎週または2週間に1回体重を測定することが推奨される2, 3。初期治療中の急速な体重増加(治療開始1ヵ月後にベースラインから5%以上増加する等)は,長期の体重増加を強く予測するため,予防や改善策を直ちに検討すべきである4-6。抗精神病薬の投与を継続する場合は,最低でも年1回体重を測定することが推奨される2, 3, 7。
実臨床では,抗精神病薬の投与を継続する場合の体重やその他の代謝性副作用のモニタリングは一貫性がなく限定的であり,推奨されるベストプラクティスには達していない8-13。
治療と予防
この分野に関連する文献のほとんどは,薬剤による治療中に増加した体重を減らす試みを取り上げたものである。現在では,早期介入により体重増加を予防または軽減できることを示唆する有用なデータがある14, 15。
体重増加が生じたときの最初の選択肢は,薬剤の切り替え,行動プログラムの実施,またはその両者である。薬剤の切り替えは再燃と治療中断のリスクを伴うが16,体重増加を改善する手段として,アリピプラゾール17, 18,ziprasidone19-21,ルラシドン22, 23への切り替えが強く支持されている。その他の体重増加を起こしにくい抗精神病薬への切り替えも有用となる可能性がある2, 24, 25。他の選択肢としてアリピプラゾールを補助投与する方法があり,クロザピンやオランザピン等の抗精神病薬にアリピプラゾールを追加すると体重が減少することが観察されている15, 26。
抗精神病薬の投与を完全に中止すると体重が減少する可能性はあるが27, 28,複数回エピソードの統合失調症患者の大部分にとっては臨床的に適切な選択肢ではない。薬剤の切り替えや増強によって体重増加を最小限にしたり,体重減少を促進したりできる可能性はあるが,全体的な作用は穏やかであり,多くの患者は体重過剰のままである。BMIを正常範囲にとどめる,または近づけるためには,生活習慣介入の追加が必要なことが多い。
体重増加に対しては様々な生活習慣介入が提案・評価されており,良好な結果が出ている2, 14, 29-32。介入法は様々であるが,食事の改善と身体活動の増加を目的とした「行動的生活習慣プログラム」が主である。RCTのメタ解析では,これらの非薬物療法的な介入で体重増加の予防と介入を行えば,大きな効果があることが示された14, 30。
薬剤による体重コントロールは,行動療法や,体重増加のリスクが低い薬剤への切り替えでうまくいかない場合,もしくは肥満による身体へのリスクが明らかで差し迫っている場合にのみ考慮すべきである。抗精神病薬による体重増加に対するいくつかの薬物治療選択肢を表1.18にアルファベット順に示した。前版で推奨されていた治療のうちいくつか(H2受容体拮抗薬等)は,その使用がエビデンスで支持されなくなったため表から除外した。

リラグルチド49, 50(3mg/日皮下注射) | GLP-1作動薬。従来は2 型糖尿病に承認され,最近では糖尿病以外の患者における抗肥満薬として承認された。体重減少のための用量(3mg/日)は糖尿病に用いられる用量(1.8mg/日以下)よりも高い。薬剤による体重増加に対してのデータは少ない。あるRCTでは,オランザピンまたはクロザピンで安定している過体重の前糖尿病患者における有意な体重減少が示されている49。他の代謝パラメータに対する有益な影響がある。忍容性は良好であるが,胃腸障害を引き起こす可能性がある。前糖尿病/糖尿病患者およびクロザピンによる体重増加において推奨される治療選択肢である。現在,他のGLP-1作動薬は糖尿病についてのみ承認され,用量範囲はさらに限られている。エキセナチドLA(週1回投与のGLP-1作動薬)はクロザピン投与患者における体重減少に有効である可能性があるが51,他の抗精神病薬については効果がない52 |
メトホルミン2, 32, 53, 54(500-2,000mg/日) | 現在,多くのデータベース(非糖尿病患者関連)でメトホルミンには抗精神病薬(主にオランザピン)による体重増加を低減したり元に戻したりする効果があることを支持している。他の代謝パラメータに対する有益な影響がある。否定的な研究もいくつかあるが,メタ解析では明確かつ有意な効果が認められている。肯定的なRCTが1件あり55,その後,自閉症スペクトラム障害(ASD)の小児および青年における継続試験56が発表されている。体重が増加した場合,糖尿病の場合,多嚢胞性卵巣症候群の場合には理想的である。メトホルミン治療によりビタミンB12欠乏症のリスクが高まることに注意する57 |

orlistat65-70(120mg 1日3回,食前/食後) | 肥満への効果は信頼性があり,特にカロリー制限との併用が有効である。薬剤による体重増加に関するデータはほとんどないが,広く臨床で使用されており,ある程度の効果がある。クロザピンまたはオランザピンによる体重増加に関する試験では,効果は男性にのみ認められた69, 70。精神疾患患者でカロリー制限を行わずにorlistatを使用した場合の効果は非常に限られる。低脂肪食の食事療法を守らないと脂肪性の下痢を起こし,経口薬の吸収不良を起こす可能性がある。概して,クロザピンによる体重増加には適切な選択肢であり,体重を減少させ,便秘を防ぐ71 |

ゾニサミド75(100-600mg/日) | 体重減少作用の特徴をもつ抗てんかん薬である。150mg/日を投与したRCTでは,SGAを服用している患者で有意に体重が減少した。別のRCT(600mg/日まで)では,オランザピンによる体重増加を緩和することが示された。鎮静,下痢,認知機能低下が最もよくみられた問題であった。推奨されない |
現在,メトホルミンは抗精神病薬による体重増加の予防と治療に最適な薬剤と考えられているが,最終的にGLP-1作動薬の方が効果が高く,忍容性も良好であることが示される可能性がある。あらゆる減量方法がうまくいかなかった,少数の稀な重度の症例では,肥満手術が奏効する場合もある33。しかし,薬剤による体重増加に対する肥満手術の有効性は明らかにされていない2。
