抗精神病薬による体重増加
心代謝危険因子の1つである体重増加は,抗精神病薬でよくみられる副作用である1。抗精神病薬による体重増加の基礎にあるメカニズムは十分に解明されていないが2,5-HT2C受容体拮抗作用,H1受容体拮抗作用,D2受容体拮抗作用,血清中レプチン濃度の上昇(ひいてはレプチン受容体の脱感作)等の因子3-5が関与していることが多い。これらの薬剤が代謝に直接作用するというエビデンスはない。体重増加は食事摂取量の増加のため生じると考えられるが,エネルギー消費量の減少が原因の場合もある6, 7。体重増加のリスクは臨床反応と関連しているようにみえるが8, 9(ただし,臨床的に重要といったほどの関連性の強さではないかもしれない10),遺伝的な基礎がある可能性がある11。体重増加は,抗精神病薬を初めて使用した患者や精神病性疾患の治療初期においてより顕著な可能性があり12-14,男性と比較して女性でリスクが高いと考えられる15, 16。
ほぼすべての抗精神病薬は体重増加と関連しているが12,平均増加量は薬剤によって大幅に異なる。使用した患者間の個人差も著しく,体重が減少する患者,体重増加がみられない患者,体重が大幅に増加する患者がいる。そのため,特定の薬剤について報告された平均増加量を知っていても,個々の患者でどのくらい増加するかは予測できないかもしれない。種々の薬剤の体重増加の相対リスクは主に短期間の試験で評価されたものである。これらの限界にもかかわらず,直接および間接メタ解析の結果から,体重増加の相対リスクに基づき抗精神病薬を3群に分類することが提唱されている17(表1.17参照)。
