向精神薬と手術
麻酔薬以外の薬剤が手術や麻酔プロセスに及ぼす影響については,研究がほとんどない1, 2。そのため実際の使用は,主に理論的考察,症例報告,臨床での使用経験,個人的な意見に基づいて行われている。したがって,この分野の推奨事項は必然的に推論に基づいたものとなる。手術時および周術期に投与を継続するかどうかは,相互に影響を及ぼす様々な要因を考慮して判断する必要がある。一般的に考慮すべき点として,以下のような事柄が挙げられる。
- 全身麻酔中に胃内容物を気道内に吸い込む危険性がある。このため,通常,手術前6時間以上は絶食とするが,水等の清澄な液体は摂取後2時間以内に胃から排出されるため,常用薬を服用するための水等は,手術の2時間以上前であれば摂取してもよい3。
- 手術時に使用する薬剤と常用薬による相互作用が起こることがあり,薬剤によっては絶対的禁忌となる。こうした相互作用には,麻酔科医が適切な麻酔薬を選択することで対処するのが通常であるが,常用薬を一時的に中断する場合もある。手術時に使用する薬剤と向精神薬の重大な相互作用には以下のようなものがある。
- enfluraneは,三環系抗うつ薬の投与を受けている症例でけいれん発作を誘発する可能性がある4-6。
- ペチジン等のセロトニン性オピオイドは,MAOIの投与を受けている症例では致死性の「興奮性」反応を誘発する可能性があり,SSRIの投与を受けている症例ではセロトニン症候群を引き起こす可能性がある4-7。
- 揮発性麻酔薬(halothane,enflurane等)はQTcを延長するため8,QT延長作用のある薬剤を投与され,QT延長のECG所見があるケースでは通常は投与すべきではない。
さらに以下の事柄が挙げられる。
- 大手術によって,電解質障害,コルチゾールとカテコールアミンの放出等の著明な生理学的変化が誘発される。
- 手術ストレスや麻酔時に用いられた薬剤によって,術後に胃内容または胃腸内容うっ滞が生じることがあるため,薬剤の経口吸収性が低下する可能性がある。
継続すべきかせざるべきか
ほとんどの場合,担当麻酔科医の同意があれば,周術期も向精神薬は継続すべきである。表13.4に手術時に向精神薬を継続することのメリットやその他の点をまとめている。

単に「絶飲食」という理由で術前に向精神薬やその他の薬剤が(誤って,および/または漫然とではあるが)中断されることも多い1。「絶飲食」とする理由は,術前準備,意識消失,術後に腸を休めるため,手術自体の結果等がある。また,多くの場合は悪心や嘔吐のために,入院中に経口薬に対して不耐性が生じることもある。向精神薬を継続する判断をした場合は,意図せず投与が中断されることがないように,医療・看護スタッフにその旨を明確に伝える必要がある。
喫煙
手術を受けて回復するまで病院で過ごす多くのケースでは,喫煙の機会はほとんどまたは全くないと考えられる。突然の禁煙は精神状態に影響を及ぼす可能性が高く,また,向精神薬を継続する場合は薬物毒性を生じる可能性がある(Chapter 11─薬物動態の「喫煙と向精神薬」の項を参照のこと)。
製剤の変更
手術に関連した様々な理由により,代替の投与経路や製剤を選択することもある。この場合は,バイオアベイラビリティも変化する可能性があるため,適切な用量および頻度で処方するよう注意する。場合によっては,経口薬を液剤として,または錠剤を粉砕して,経鼻胃管(NG),経皮内視鏡的胃瘻チューブ(PEG),空腸瘻チューブから投与することもある。バイオアベイラビリティの問題は,薬剤が投与チューブの素材に吸着することでも起こりうる。
向精神薬中止に伴うリスク
- 再発(特に数日以上投与を中止した場合)38。
- 病状の悪化。例えば,リチウム39や抗うつ薬40あるいは抗精神病薬41を急に中止すると双極性感情障害の転帰が悪化する。
- 自殺。抗うつ薬を中止すると自殺リスクが高くなるおそれがある42。
- 離脱症状。周術期の診断が複雑になる可能性がある。
- せん妄。抗精神病薬43や抗うつ薬6を中止した症例に多くみられる。
向精神薬継続に伴うリスク
- 麻酔薬や周術期に投与される薬剤との,薬物動態学的相互作用や薬力学的相互作用の可能性。
- 出血の可能性の上昇(例:SSRI)44。
- 低血圧/高血圧(向精神薬によって異なる)23, 24。
- 深部体温への影響(例:フェノチアジン系薬剤)。
