精神科における静注用製剤
静注(IV)は,精神科以外では薬剤投与の主な非経口投与経路の1つである(表14.8参照)。静注の利点には,速やかな作用発現,正確な漸増,患者毎の投与計画,肝代謝の回避等がある。欠点としては過敏反応,副作用の発現頻度,感染リスク,全体的なコストの増加等が最も多く議論されている。他の医療分野と異なり,精神科では静注の利用度が著しく低い。その証拠に,英国で2013年にPrescribing for Mental Health Observatoryが実施した監査では,記録されていた急性障害のエピソード2,172例のうち,静注用製剤が用いられたのはわずか2例にとどまっていた1。
静注経路による向精神薬の投与は主に急性の行動障害の管理に焦点が当てられてきたが,その適応には幅広い診断が含まれ,とりわけ感情障害や不安障害もある。静注用製剤を安全に投与するには特別な注意と専門知識が必要であることから,静注は,標準治療によって望ましい効果が得られなかった場合に,特別な臨床状況下(訓練を受けたスタッフが配置され,最適なモニタリング,蘇生装置,人工呼吸器のすべてをすぐに利用できる総合病院等)で検討されることが最も多い2。静注でのみ投与が可能な薬剤もある(例:brexanolone)。
