遅発性ジスキネジアの治療
遅発性ジスキネジア(TD)は,過去数十年間と比べて現在ではみかける頻度がやや低下している問題であり1, 2,これはおそらく,総じてFGAより遅発性ジスキネジアのリスクが低いSGAが導入され,広く使用されているためである3-6。遅発性ジスキネジアと確定診断された患者の治療はうまくいかない場合が多いため,予防,早期発見,早期治療が最も重要である7, 8。遅発性ジスキネジアは認知機能障害の増大7, 9,精神病理の重症度増加10, 11,死亡率の上昇12, 13に関連することを示唆するエビデンスがある。
SGAは遅発性ジスキネジアを引き起こす可能性が低いが14-19,発生自体はしており,全発生率は約4%と推定され18,リスクは個々の薬剤によって異なる20-23。あるメタ解析では,遅発性ジスキネジアの年間リスクはFGA投与患者において3.7-12.5%(薬剤によって異なる),SGA投与患者において1.7-4.8%と推定された24。遅発性ジスキネジアの発生リスクは,薬剤によるD2受容体占有の程度に関連する可能性がある(占有率が大きいほどリスクが高い)25。しかし,関連するRCTの広範なメタ解析によるデータでは,SGAによる遅発性ジスキネジアのリスクがFGAより低いことは,FGAの高用量での使用と関連があるという考え方は支持されなかった24。ドパミン受容体部分作動薬(または少なくともアリピプラゾール)は,遅発性ジスキネジアのリスクが最も低い可能性が示唆されている24。遅発性ジスキネジアのリスクがFGAのLAIとSGAのLAIとで異なるかどうかは不明であるが26,SGAのLAIの方が発生率は低いであろうと推測される。
遅発性ジスキネジアは低用量のハロペリドールによって(かつ急性運動障害の既往がない場合に27)発生することや,メトクロプラミド等の他のドパミン拮抗薬の使用後に発生することがある28。
遅発性ジスキネジアは,初回エピソード29, 30と,確定診断された統合失調症31の双方の薬物治療歴のない患者においても認められている。
治療─最初のステップ
関係当局の多くが,遅発性ジスキネジアの初期徴候が認められる場合の最初の段階として,併用している抗コリン薬の中止と抗精神病薬の減量を推奨している32, 33(ただし,減量により最初は遅発性ジスキネジアが悪化することがある)。しかし,コクランは減量34または抗コリン薬中止35をあまり支持しておらず,米国神経学会は減量を推奨していない36。それでも,遅発性ジスキネジアが初めて認められたときには処方されている抗精神病薬を中止し,遅発性ジスキネジアのリスクが低いと認識されている別の薬剤で置き換えるのが一般的なやり方である。しかし,特定のSGAへの切り替えの有益性を示すエビデンスはあまりない36。クロザピン32, 37の使用が,おそらくこの点では最も強く支持されているが,もう1つの弱い線条体ドパミン拮抗薬であるクエチアピンも有効となる可能性があり38-44,オランザピン39-42, 45, 46とアリピプラゾール47もさらなる選択肢の候補である。リスペリドンについてもいくらかの支持するデータがあるが48,リスペリドンはクロザピン,オランザピン,クエチアピンと比較して急性運動障害に関連する可能性が高いことから,遅発性ジスキネジアと確定診断された患者において論理的な選択ではないであろう。
治療─追加薬剤
遅発性ジスキネジアの治療として減量を推奨する十分なエビデンスは存在せず,抗精神病薬の切り替えまたは中止が必ずしも有効または妥当であるとは限らないことから,追加薬剤がしばしば使用される。2020年のメタ解析49では,承認されている3つのVMAT-2阻害薬,ビタミンE,アマンタジン,ビタミンB6(ピリドキシン)のみに,明らかな有益性が認められた。遅発性ジスキネジアに対して最もよく処方される追加薬剤を表1.15に示す。

deutetrabenazine8, 52, 55-57 | deutetrabenazine(VMAT-2阻害薬)もTDの治療薬として有効である。米国ではTD治療薬として承認されている58。テトラベナジンよりエビデンスの支持は強い。テトラベナジンより半減期が長いが,それでも1日2回摂取が必要である。精神医学的作用や神経学的作用の発現率は低い。用量は12-48mg/日 |

治療─考えられるその他の選択肢
遅発性ジスキネジアに対して提案されている治療の数が多いことは,標準治療の有効性がやや限定的であることの表れであることは間違いなく,少なくともvalbenazineとdeutetrabenazineの導入前ではそういえたであろう。こういった治療選択肢を表1.16にアルファベット順に示す。

英国で承認されている3つのVMAT-2阻害薬のうち,本邦ではテトラベナジン(コレアジン®)のみ処方できるが,保険適用外であり,適応症が「ハンチントン病に伴う舞踏運動」という希少疾患に対する薬剤であることから,副作用の確認が行えるよう,処方する際には製薬会社に連絡する必要がある薬剤である。
