ケタミン
背景
非競合性N-メチル-D-アスパラギン酸(NMDA)受容体拮抗薬であり解離性麻酔薬であるケタミンは,この20年間で,新規の有効な即効性抗うつ薬として注目されるようになった。2000年に,Bermanらは,大うつ病性障害(MDD)患者に麻酔域下用量のケタミン(0.5mg/kgを40分かけて)を単回静脈内投与(IV)した大規模なRCTの結果を報告した1。ケタミンは投与後数時間以内に有意な抗うつ効果を示し,その効果は投与後3日まで漸進的に増大した。以降,複数の試験で,単極性うつ病,双極性うつ病のいずれにおいても(治療抵抗性の患者を含め)同様の結果が示されている2-6。
ケタミンは光学異性体であるS体とR体(esketamineとarketamine)が等量含まれるラセミ混合物であり,esketamineの方が強力にNMDA受容体に結合する。ケタミンは現在,治療抵抗性うつ病(TRD)に対しては依然として適応外使用であるが,esketamine点鼻薬(SpravatoTM)が開発され,欧州および米国でTRDに対する使用(経口抗うつ薬との併用)が承認されている。
作用機序
現在のところ,ケタミンおよびesketamineが有する即効性抗うつ効果の正確な作用機序は明らかにされていないが,グルタミン酸作動性ニューロンからのグルタミン酸放出を通常抑制する作用をもつ,γ-アミノ酪酸(GABA)作動性介在ニューロン上のNMDA受容体の遮断を介してもたらされると提唱されている7。この抑制解除により,大脳皮質で急激にグルタミン酸濃度が上昇し,シナプス後α-アミノ-3-ヒドロキシ-5-メチル-4-イソオキサゾールプロピオン酸(AMPA)受容体が活性化し,シナプス形成および神経可塑性に関与する経路の下流に作用する7。
投与経路
TRDに対するケタミンの最適な投与方法は十分に確立されていないが,承認されたesketamine点鼻薬の投与ガイドラインがある(表3.6)。適応外使用ではケタミン静注(0.5mg/kgを40分かけて)がゴールドスタンダードであり,有効性を支持する最良のエビデンスがある。皮下(SC),筋肉内(IM),経口,舌下(SL)など,他の投与経路も提案されているが,これらの投与経路の相対的な有効性と安全性,症例毎の最適な投与レジメンを評価するためには,さらに研究が必要である。各投与経路には,バイオアベイラビリティ,効果の持続時間,実用性,患者の快適度の点でそれぞれの利点と課題がある。ケタミンは様々な投与経路と用量が試験されているものの,投与戦略が確立していないが,入手できるエビデンスと臨床経験を考慮した推奨用量を表3.6にまとめる。

副作用
ケタミンは,抗うつ効果をもたらす用量で投与すると,概して重大な解離性症状を引き起こす8。知覚変容もみられ,重大な不安が発現することもある。したがって,ケタミンを投与する場合は,投与中および投与後1時間,熟練した臨床医が患者を必ず観察する必要がある。また,稀ではあるが,ケタミンは咽頭けいれんを誘発することが報告されているため,観察する臨床医は中級または上級救命処置の訓練を受けた者でなければならない。低用量のケタミンを経口または舌下投与する場合は,強い解離性症状を誘発する可能性が低いため,一度,臨床医の監督下で試験的投与を行った後は医療機関外(自宅)での投与も可能と考えられるが,投与後少なくとも1時間は,患者に車の運転,機械操作,その他の危険度の高い活動を行わないよう指示する必要がある。さらに,処方医は,薬剤の娯楽目的での流用や違法な使用のリスクを考慮しなければならない。
ケタミンは血圧および心拍数に重大な影響を及ぼす場合があるため,投与前に,血圧測定,全血球数,肝機能検査,甲状腺機能検査,ECGを含めた身体診察を行うことが推奨される。また,ケタミンの投与中および投与後に身体モニタリング(血圧および心拍数)を行うことも望ましい。
