性機能障害
一次的な性機能障害は一般的にみられるが,信頼性のある標準的データは不足している1。身体疾患,精神疾患,物質乱用,処方されている治療薬のいずれも性機能障害の原因となりうる2。性機能障害の有病率は,一般集団の30%4と比較して統合失調症では30-82%3と推定されているが,一般集団でも統合失調症患者でも,データ収集法によって有病率が異なることに注意しなければならない(自発的な報告は少ないが,機密保持を保証した質問紙調査では報告数が増え,直接質問するとさらに報告数が増える2)。精神病症状のある患者を対象とした研究では,自発的に報告する患者は37%であったが,直接質問した場合には46%で問題があると訴えている5。
性機能障害はQOL6や服薬コンプライアンスに影響を与えるため(性機能障害は治療中断の大きな原因の1つとなる)7, 8,可能であれば薬物療法開始前の性機能を調べておくべきである(質問票が有用と考えられる)。性機能障害の訴えがある場合は,原因となっている身体状態あるいは精神科的状態が進行していること,またはそれらに対する治療が不十分であることを示している可能性がある9, 10。性機能障害は薬物治療が原因のこともあり,その場合は介入によりQOLが大きく改善する可能性がある11。

精神病による影響
性機能障害は,統合失調症の初回エピソードにおける既知の現象であり14, 15,疾患が確定した患者での割合は男性で82%,女性では96%に上り,QOLの低下と関連する6。薬剤を投与されていない患者でも有病率は高い(17-70%)ことから16,抗精神病薬の副作用のみが原因であるわけではない。男性では性的欲求の減少,勃起障害,早漏の訴えが多く17,女性は性的快感の減少の訴えが多い17, 18。精神病症状のある女性は妊娠しにくいことが知られている19。精神病症状があると良好な精神的・性的関係を築くことが難しくなり,抗精神病薬による治療が性機能障害を改善する場合もある20。精神病症状があると性機能の評価が難しいことは明らかであろう。この点で,Arizona Sexual Experience Scale(ASEX)は有用かもしれない21。
抗精神病薬の影響
性機能障害はほとんどの抗精神病薬で副作用として報告されており,従来型抗精神病薬の投与を受けている患者では最高45%が性機能障害を経験する22。抗精神病薬による影響の感受性は患者によって異なり,どの影響も可逆的である。しかし,身体疾患や抗精神病薬以外の薬剤も性機能障害を起こす場合があり,これらについて考慮していない研究も多く,種々の抗精神病薬における性機能障害の有病率を比較することは難しい23。
抗精神病薬はドパミン伝達を減少させる。このこと自体がリビドーを減少させうるが,同時にネガティブフィードバックでプロラクチン値を上昇させる可能性もある。複数の研究において高プロラクチン血症と性機能障害の関連が示されており24,抗精神病薬に伴う性機能障害の40%はプロラクチン値の上昇によると推定される4。また,高プロラクチン血症は女性で無月経,男性と女性で乳房肥大と乳汁漏出を起こすことがある25。抗精神病薬が性機能障害の原因となる全体的な傾向は,プロラクチン増加の傾向(リスペリドン>ハロペリドール>オランザピン>クエチアピン>アリピプラゾール9, 23, 26)と関連することが示唆されている。ただし,CUtLASS-1研究では,性機能障害に対してFGA(主にスルピリド。プロラクチン値上昇と関連することが知られている他のFGAも含む)がSGAより悪影響を及ぼさないとしていることに注意すべきである(SGA群の70%の患者がプロラクチン値上昇と関連のない抗精神病薬の投与を受けていた)。実際は1年間の試験期間において,いずれの群でも性機能は改善していた20。アリピプラゾールは単剤の場合27も他の精神病薬と併用した場合28, 29も,性機能に関する副作用は比較的ないといえる。cariprazineは理論的に適切な代替薬となる(切り替え試験はまだ報告されていない)30。
抗コリン作用は性的興奮を阻害し31(併用している抗コリン薬が性機能障害の原因となる場合がある32),末梢α1受容体を遮断する薬剤は特に男性の勃起障害,射精障害を起こす可能性がある11。末梢α1受容体およびコリン受容体に対する拮抗薬は,持続勃起症の原因となる可能性がある33。抗精神病薬による鎮静と体重増加は,性的欲求を減少させることがある33。このような原則は,様々な抗精神病薬の性機能に関する副作用の予測に有用である(表1.31参照)。精神病症状をよりよくコントロールする抗精神病薬に切り替えること自体が,性機能障害に対して有効となる可能性があることを念頭に置いておく。

治療法
性機能障害を治療する前に,可能性の高い原因について詳細な評価を行うことが必須である。身体疾患(糖尿病,高血圧,心血管疾患等)の可能性がない場合,または治療が行われている場合(例:肥満73)には,次の原則を当てはめる。
性機能障害は自然に寛解する場合もあるが33,寛解するとしても明らかになるまでに6ヵ月かかることもあり30,そのような寛解は,抗精神病薬そのものへの忍容性よりも,疾患の重症度の低下に関連している可能性が高い。症状が持続する場合,第一段階は,原因となっていることが明らかな薬剤を減量あるいは中止することである。次の段階は,患者が経験した種類の性機能障害を起こす可能性が低い薬剤へ切り替えることである(表1.30参照)。他の選択肢として,アリピプラゾール5-10mgを追加することもあり,アリピプラゾールでプロラクチンが正常範囲内になると性機能が改善する74-77。アリピプラゾールを追加しても奏効しない場合,またはこの方法が実用的でない場合は「副作用止め」を試してみる。例えば,シプロヘプタジン(4-16mg/日で5-HT2受容体拮抗薬として作用する)は,SSRIによる性機能障害の治療に用いられているが,副作用として鎮静が起こりやすい。FGAを投与された患者において,ミルタザピン(5-HT2受容体拮抗薬およびα2受容体拮抗薬でもある)がオーガズム機能不全を緩和する可能性があることを示す若干のエビデンスが認められる78。アマンタジン,bupropion,buspirone,ベタネコール,yohimbineも用いられているが,効果のレベルは様々であり,予期せぬ多くの副作用や薬物相互作用が問題になることがある。高プロラクチン血症が性機能障害の原因となっている可能性を考え,セレギリン(ドパミン作用を増強)を試したRCTがある。しかし,この試験では否定的な結果であった79。テストステロンパッチは女性のリビドーを増加させるが,乳癌のリスクが有意に上昇するおそれがあることに注意しなければならない(表1.32)80, 81。

「副作用止め」の投薬を支持するエビデンスベースは乏しい33, 101。肯定的な試験でも,症例数が少なく,試験期間が短く,交絡因子(年齢,併用薬,投与開始前の性機能障害以外の理由による抗精神病薬の切り替え)の調整が行われていなかったことから,その結果の一般化可能性は限られていることに注意する101。試験間のデータの比較は,転帰の測定に様々な評価ツールが用いられていたために,さらに複雑である102。
シルデナフィル(Viagra)やアルプロスタジル(Caverject)は,勃起障害の治療に対してのみ有効である(リビドーや中枢神経の興奮に対する効果はない)。精神的健康に問題のある患者では,性機能障害の治療を行うクリニック等で行われる心理的アプローチの使用は困難である場合がある11。
