肥満手術における向精神薬
肥満手術を受けた症例では,精神疾患が比較的多い1。肥満手術を受けようとする患者の 1/3超が向精神薬を処方されている2。肥満手術は臨床的に重要な薬物動態の変化を伴う可能性があるが,向精神薬がどのような影響を受けるのかは,個人差やデータが限られていることから,予測が難しい。現在のところ,治療の綿密なモニタリングと,術後の継続的な症状のモニタリングの必要性が研究により支持されている3。
肥満手術の術式は以下のように分類される。
- 主に摂食量を制限する手術:スリーブ状胃切除術および胃バンディング術
- 主に栄養の吸収を抑制する手術:胆膵バイパス術および空回腸バイパス術
- 摂食量制限と栄養吸収抑制の組み合わせ:ルーワイ胃バイパス術(RYGB)および胃縮小十二指腸スイッチ術(gastric reduction duodenal switch:GRDS)
栄養の吸収を抑制する手術(RYGB およびGRDS を含む)の方が,薬剤の吸収を変化させる可能性が高い。データの大半は,RYGB を受けた症例に関する研究から得られたものである。これらのデータが他の術式による結果とどのように関連するかは不明である。
肥満手術後の薬物動態の変化
いずれの術式でも以下の変化が起こる可能性がある。
- 胃内 pH と撹拌の変化による,錠剤の崩壊および溶出時間の変化
- 胃内容排出速度の変化による,薬物吸収率の変化
- 脂肪組織の減少(特に脂溶性の薬剤)および蛋白結合の変化による,薬物分布の変化
- 体重減少後の肝機能改善による,薬物代謝の変化
- 体重減少後の腎機能の変化による,薬物排泄の変化
栄養の吸収を抑制する術式では,以下の変化も起こる可能性がある。
- 薬物吸収面積の減少(吸収機能のある腸管長の短縮)
- 親油性薬剤の可溶性の変化(近位小腸にて胆汁酸の修飾を受けない)
- 腸管長の短縮による,腸壁での薬物代謝の減少
剤形
薬剤の崩壊および溶出を遅延させる剤形はいずれも,肥満手術後には薬物吸収を低下させる可能性がある4。手術の前に即放性製剤に切り替えることが一般的に推奨される4, 5(客観的データよりも専門家のコンセンサスに基づく推奨である6)。口腔内崩壊製剤や液体製剤は崩壊の段階がないため,固形の錠剤で吸収の低下が懸念される場合には好ましい可能性がある7。非常に大きい錠剤(例:直径 10 mm 超)は,摂食量を制限する手術後には通過できないことがあるため,避ける必要がある。

概要
- 肥満手術後の気分安定薬使用に関する文献は,数件の症例報告に限定されている。リチウムは治療域が狭いため,使用する場合は特に注意が必要である
- 肥満手術後は,経口避妊薬の吸収が低下する可能性がある41。催奇形作用がある気分安定薬を処方されている患者には,経口薬以外の避妊法が推奨される
肥満手術前後におけるリチウム
肥満手術前後にリチウムの使用を継続する場合は,特に綿密なモニタリングが必要となる。以下の指針は,現時点で得られている症例報告および専門家の見解に基づいている40。
- 血漿中リチウム濃度は,手術前および手術後6週までは週1回(水分摂取量が徐々に増加するため),手術後6ヵ月までは2週に1回,その後は月 1 回モニタリングする。肥満手術後1年の時点で,通常のリチウムのモニタリングを再開する。
- 血漿中リチウム濃度が25%を超えて上昇した場合,または1.2mmol/Lに近づいた場合は,リチウムの減量を検討する。
- 毒性の徴候がみられる場合はリチウムの使用を中止し,用量を見直す。
- 正規の評価尺度を使用して,精神状態を定期的にモニタリングする。
- 手術前の期間には,1日に2.5-3Lの水分の摂取を奨励する(流動食への置き換えを含む)。

肥満手術後における体重増加のリスクを伴う向精神薬
10-20%の症例では,肥満手術後に体重が大きく再増加すると推定されている49。体重増加に関連する向精神薬が肥満手術後の転帰にどのような影響を及ぼすかについては明らかでないものの,リスクの高い薬剤の使用は避けた方がよいであろう。コントロール不良の精神疾患は体重再増加の危険因子であることを示すエビデンスがあるため,各患者の臨床的環境を考慮する必要がある(特に,治療により安定していて再発のリスクが高い場合)49。
アルコール50, 51
胃バイパス術は,アルコール吸収の加速,最高血中アルコール濃度の上昇,アルコール消失の遅延を伴う。また,胃バイパス術後はアルコール使用障害のリスクも上昇する。スリーブ状胃切除術に関しては明確なデータが得られておらず,胃バンディング術により変化がみられるというエビデンスはない。
より広い考察52
臨床では,多くの患者が手術後に投薬の大きな変更を必要としないであろう。手術後の症状再発は,薬物動態の変化に関連していない可能性がある。手術後は精神衛生の改善が見込まれるものの,期待通りの体重減少が達成できない,忍容性不良,不満足等,様々な要因により悪化する場合もある。
