推奨─ AF における向精神薬

状態 提案される薬剤 避けるべき薬剤
統合失調症/統合失調感情障害
これら自体が AF リスクの上昇に関連する可能性がある10
1件の症例対照研究では，抗精神病薬によりAFのリスクが17％上昇することが示唆された11		 発作性または持続性 AF の場合には，cariprazine，ブレクスピプラゾール，ルラシドンが適切な選択肢の可能性がある
心拍数がコントロールされた永続性 AF の場合には，薬剤の選択はそれほど重要ではないが，ECG に強力な影響を与える薬剤（例：ziprasidone，ピモジド，sertindole）と心拍数を増やす薬剤は避けた方がよい		 クロザピン12, 13，オランザピン14, 15，アリピプラゾール16, 17，パリペリドン18の使用での AF が報告されている。因果関係ははっきりと確立したものではないが，孤立性，発作性または持続性 AF では使用しない
虚血性心疾患では QT 延長作用のある薬剤は使用しない（QT 延長に関する項を参照）
抗精神病薬と AF の関連性11は，代謝障害による可能性があるが19，関連性がないことを示唆している研究もある20
双極性障害 バルプロ酸
リチウム
カルバマゼピン		 気分安定薬は AF のリスクに影響を与えないと考えられる
バルプロ酸は房室伝導ブロックを引き起こす可能性がある21
リチウムの過量投与後に AF がみられた症例 1 例22，慢性中毒例において AF がみられた症例 1 例23が報告されている
うつ病
未治療のうつ病は AFの再発を招くことがある24
AF によりうつ病および不安のリスクは上昇する25		 SSRI，ただしワルファリンやその他の抗凝固薬との相互作用26により重度の出血のリスクが高まるため27，注意する
動物実験では SSRI の抗不整脈作用が示唆されている28, 29
パロキセチンにより，一連のうつ病のない症例で発作性AFが改善した30
ベンラファキシンは心房伝導に直接影響を与えることはなく31，発作性 AF に除細動作用を有するかもしれない32
抗うつ薬投与開始後に AF 発生率は低下する33, 34
agomelatineが心伝導や凝固に影響を与えるというエビデンスはない		 冠動脈疾患では三環系抗うつ薬は避ける35
三環系抗うつ薬は AF を誘発する可能性があるが36, 37，ワルファリンと併用しても出血リスクを高めない26
データベース研究では，一般的に抗うつ薬は AF リスクを高めないことが示唆されている38
不安障害
（不安症状は AF リスクを高める）39		 ベンゾジアゼピン系薬剤

プレガバリンに関連した AF の症例報告が 1 例ある40
アルツハイマー病 アセチルコリンエステラーゼ阻害薬（AChE-I）［ただし，発作性「迷走神経依存型」AF（心拍数低下により誘発された発作性 AF）の場合は徐脈作用に注意する］
リバスチグミンは相互作用の可能性が最も低い
メマンチン		 発作性「迷走神経依存型」AF ではコリンエステラーゼ阻害薬は避けること
